Main St. <br /> Stockton, CA., 95202 <br /> On March 16, 1998, San Joaquin County Public Health Services, Environmental Health Division (PHS- <br /> EHD) advised you that current and back fees were due by April 26, 1998 for an illegally abandoned <br /> underground storage tank (UST). The UST is located in the southern sidewalk of Main Street, at the <br /> northwestern edge of the property line of the above referenced site. The March 23, 1998 statement <br /> listed $2066.50 due for UST fees and they have not been received as of November 1, 1998. <br /> On November 6, 1998, PHS-EHD again mailed you an official notice that the fees and required UST <br /> Closure Plan for the illegally abandoned UST were past due. A deadline of December 1, 1998 was <br /> established for the submittal of the fees and the closure plan. As of the date of this notice, none of <br /> the requirements for the illegally abandoned UST have been submitted. <br /> Enclosed please find copies of the original March 16, 1998 inspection report, the March 23, 1998 UST <br /> billing statement, a UST fee worksheet with an explanation of the fees due, PHS-EHD correspondence <br /> dated November 6, 1998, and a blank UST Closure Plan and Service Request form. <br /> This UST is in violation of the requirements of the California UST Regulations and cannot remain in <br /> the ground. You are in non-compliance with directives issued by PHS-EHD. Failure to comply <br /> with directives issued by PHS-EHD and remaining in non-compliance with UST regulations places you <br /> in jeopardy of becoming ineligible to receive reimbursement from the State Water Resource Control <br /> Board (SWRCB) Cleanup Fund (CUF). <br /> You are hereby directed to submit the closure plan, complete with all current and back fees, to <br /> PHS-EHD by December 21, 1998.
