Bomb Threat Checklist <br /> Date and time threat received <br /> How the threat was received (phone, mail, etc.) <br /> If the bomb threat is received by mail or in person, do not handle letter, envelope, or <br /> package. Immediately inform your supervisor. <br /> If the threat is by telephone, record exact words of the caller. <br /> Questions to ask: <br /> 1. When is the bomb going to explode? <br /> 2. Where is the bomb located? <br /> 3. What kind of bomb is it? <br /> 4. What does it look like? <br /> 5. Why did you place the bomb? <br /> 6. Where are you calling from? <br /> Description of the caller's voice: Male Female Young <br /> Middle Age Old Accent Nervous Excited <br /> Intoxicated Laughing Speech Impediment <br /> Other <br /> Is the voice familiar? If so, who did it sound like? <br /> Any background noise on the phone? <br /> Time caller hung up <br /> Any additional information <br /> This form is to be filled out by the employee who receives the bomb threat. Give this <br /> form directly to a police officer after it is completed. Do not discuss this information with <br /> any of your co-workers. We want to protect the integrity of each person's statement. <br />
