13. Documentation <br /> 14. Map to Hospital <br /> I. KEY PERSONNEL <br /> Mr. Ken Wendt is the Operations Mgr(OM), Mr. Don Gilmer is the Project Manager (PM), and <br /> Mr. Greg Taylor is the Site Safety Officer (SSO). <br /> The site safety officer and site workers have completed 40 hrs of comprehensive health and <br /> safety training which meets the requirements of title 29 Code of Federal Regulations Part 1910.120. <br /> The site workers will also receive site specific training before entry to the site. <br /> The Project Manager is responsible for generating and organizing the SSHP, which describes all <br /> planned field activities and potential hazards that may be encountered at the site. The PM is also <br /> responsible for assuring that adequate training and safety briefings for the project are provided to <br /> the project team. Copies will be available to other contractors prior to the start of field activities. <br /> Short service employees (those who have been employed less than 6 months) will be assigned a <br /> mentor who will train and supervise the employee. Short service employees will be identified by <br /> wearing a green hard hat. <br /> The Site Safety Officer is responsible for ensuring that all data acquisition is performed in <br /> accordance with the work plan and SSHP. <br /> The Site Safety Officer's responsibilities include: <br /> 1. Following the SSHP <br /> 2. Reporting to the PM and/or OM any unsafe conditions or practices. <br /> 3. Reporting to the PM and/or OM all facts pertaining to incidents which <br /> result in injury. <br /> 4. Reporting to the PM and/or OM safety equipment malfunctions or <br /> deficiencies. <br /> 5. Providing site safety briefings on a daily basis. <br /> 6. Inspecting all personal protective equipment prior to on-site use. <br /> 7. Enforcing the "Buddy System" as appropriate for site activities. <br /> 8. Posting the telephone numbers of local emergency services. <br />
