This certification is required to be submitted within 30 days of <br /> completion of work to the California Environmental Reporting System(CERS). Immediately log into CERS, complete <br /> the required information, and submit for review by the EHD. <br /> This is a minor violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. No records were available for the <br /> work done during the construction at the site. These records shall be maintained on site for at least three years. <br /> Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br /> maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br /> field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) <br /> results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring <br /> records for the last three years on site and submit copies to the EHD. <br /> This is a minor violation. <br /> 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a minor violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the May 2015 <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> missing alarms include all sensors on April 8, 2015 (fuel alarms and sensor out alarms) and April 10, 2015 (sensor out <br /> alarms). The May 2015 report stated that the April 15, 2015 UDC alarm was due to rainwater- it was due to testing <br /> conducted that day. During the monthly inspection, the designated operator shall review the alarm history for the <br /> previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the <br /> alarm history with documentation taken in response to any alarms to the monthly report.
