Dr. Martin Luther King Jr. Blvd., Stockton Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I, CLASS II,or MINOR-Notice to Comply) <br /> This is a routine and monitoring certification inspection. Observed the following violations: 11,17,23 <br /> 11. The 87 and 91-octane STP sump sensors were found not located at the bottom of the tank to detect <br /> a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak <br /> at the earliest possible opportunity. Immediately relocate the sensors to the lowest point of the sumps. <br /> Technician corrected the violation on site. <br /> 17. The 87 and 91-octane line leak detectors failed to detect a leak when tested. All line leak detectors <br /> shall be capable of detecting a 3-gallon per hour leak at 10 psi. The service technician adjusted the 91- <br /> octane 99-LD2000 leak detector, retested it, and it passed. Immediately have a properly licensed, <br /> trained, and certified contractor repair or replace the failed 87-octane leak detector (LG 113-x listed, if <br /> applicable) and obtain a permit within one business day from the EHD. If the failed leak detector can't <br /> be replaced immediately, there is a possibility that the UST system may be red tagged to prevent fuel <br /> inputs. <br /> 23c. The June 2011 designated operator monthly inspection report was not found on site. Designated <br /> operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and <br /> ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br /> maintained on site. Submit copy of the June 2011 report to the EHD by August 31, 2011 . <br /> NOTES <br /> Technician tightened the nipple riser assembly and jack screw in the 87-octane spill bucket before the <br /> spill bucket could pass the one hour hydrostatic test. <br /> Please be aware that submission of your annual financial responsibility certification and chief financial <br /> officer letter to EHD is required. Received a copy of both documents on site. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by August 31, 2011 <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> T . i FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EH In pec t Received By: v Title: <br /> EH �� �.voU11211, CONTINUATION FORM <br />
