PUBLIC H?ALTH S <br /> SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DIVISION ` <br /> Karen Furst, M.D., M.P.H., Health Officer P <br /> 304 East Weber Avenue, Third Floor • Stockton, CA 95202 q <br /> 209/468-3420 <br /> September 1, 1999 <br /> Craig Bell <br /> Fillner Construction, Inc. <br /> 3633 Seaport Blvd. <br /> West Sacramento CA 95691 <br /> RE: 440 W Charter Way, Stockton CA <br /> San Joaquin County Public Health Services Environmental Health Division (EHD) has <br /> performed a preliminary review of your submittal for application to install underground <br /> storage tanks at the above referenced site. <br /> P ase submit the following information so that I may continue processing your application: <br /> 1) One additional complete set of drawings illustrating the locations of all existing and <br /> proposed structures, including existing UST's in non-erasable print to the EHD.Upon <br /> approval, one set will be retained by the EHD and the other two will be returned. <br /> 2) Equipment and material specification sheets and manufacturer's brochures describing <br /> installation instructions, field testing procedures, operating and monitoring systems and <br /> listing numbers or other evidence of a nationally recognized testing organization approval. <br /> All proposed leak detection methods and equipment must meet the requirements <br /> specified in LG 113 -13 and have been reviewed by the State Water Resources Control <br /> Board Staff. <br /> L""/An emergency response plan in accordance with CCR Title 23, Division 3,Chapter 16 <br /> Section 2632 (d)(2)A response plan which demonstrates,to the satisfaction of the local <br /> agency,that any unauthorized release will be removed from the secondary containment <br /> system within the time consistent with the ability of the secondary system to contain the <br /> hazardous substance, but not more than 30 calendar days or a longer period of time as <br /> approved by the local agency.The response plan shall include but is not limited to the <br /> following: <br /> A) A description of the proposed methods and equipment to be used for removing <br /> and properly disposing of any hazardous substances, including the location and <br /> availability of the required equipment if not permantly on-site, and an equipment <br /> maintenance schedule for the equipment located on-site: <br /> (B) The name(s) and title(s) of the person(s) responsible for authorizing any work <br /> necessary under the response plan. <br /> (or) <br /> Complete the enclosed emergency response plan form <br /> A Division of San Joaquin County Health Care Services <br />
