The report was submitted by WHF <br /> Environmental Consultants, Inc, Please review the attached <br /> correspondence from the CVRWQCB and respond to all the items <br /> listed. In addition to the CVRWQCB concerns, PHS/EHD has the <br /> following concerns and recommendations: <br /> 1. An aeration permit was granted by San Joaquin County Air <br /> Pollution Control District. Please summarize activities <br /> to date of all remediation activities. <br /> 2 . Submit a work plan and permit application to define <br /> remaining soil contamination; include borings located <br /> east of MW4S and MW4D as noted in the CVRWQCB <br /> correspondence. <br /> 3. Submit a copy of the required reports to the CVRWQCB at <br /> the time of submission to PHS/EHD. <br /> 4 . Submit the first quarterly report to PHS/EHD and CVRWQCB <br /> by September 30, 1991, and follow with reports every <br /> three months. The reports are required pursuant to <br /> Section 2652 (d) of the Underground Storage Tank <br /> Regulations (California Administrative Code, Title 23, <br /> Subchapter 16) . These reports are to be submitted until <br /> the investigation and cleanup of the site is deemed <br /> adequate. The reports are to show the: <br /> -status of investigation and cleanup activities including <br /> the results of all investigations completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, scheYules for <br /> the completion of the investigation of the site and <br /> remediation; <br /> A Division of San Joaquin Counry]icaith Care Services <br />
