The site map was missing exits, emergency response <br /> equipment, evacuation staging area, loading areas, storm drains, and utilities shut offs. Please add the missing <br /> elements to the site map. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer <br /> drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and <br /> storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list <br /> it on the map and label as"NA". <br /> This violation was corrected on site. <br /> This is a minor violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually. The hazardous material business plan was submitted late on 2/28/2013 and 11/21/2019. <br /> Only the facility section was submitted on 8/5/2015, 1/14/2015, and 2/28/2013.All three parts were submitted on <br /> 11/21/2019. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year. <br /> This violation was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> All violations were corrected on site. Areas that were inspected: the control room, pump room, diesel tank, and <br /> the sodium hypo chloride tank areas were inspected. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
