On the 10/25/18 designated operator(DO)inspection report under -Section III The DO failed to add a description to all the components marked N/A or N from section VII through XI -Section I The DO failed to write the CERS ID -Section IX The DO failed to mark Yes, No or N/A to the question"Are all sensors in under-dispenser containment located to detect a leak at the earliest opportunity?" The DO failed to mar YES, NO or N/A under the question"Are all sensors in containment sumps inspected located to detect a leak at the earliest opportunity?" -Section XI The DO failed to mark Yes or No to the question"Have all individuals performing facility employee duties received the required facility employee training within the past 12 months?" On the 12/15/18 designated operator(DO)inspection report under -Section III The DO failed to add a description to all the components marked N/A from section VII through -Section X The DO failed to state the correct date for overfill prevention equipment inspection testing. He stated that it was not done, however it was done on 11/15/18. Furthermore the alarm history stated 12/15/18 however the DO report stated 12/26/18. On the 3/29/19 designated operator(DO)inspection report under -Section VI The OWNER/OPERATOR failed to provide at least one of the following: name, signature, and date within 48 hours of receiving the DO report under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES section. -Section VII The DO fail to mark yes or No for the question"Has each follow-up action of section III from the previous inspection been completed appropriately?" On the 4/29/19 designated operator(DO)inspection report under -Section VI The OWNER/OPERATOR failed to provide at least one of the following: name, signature, and date within 48 hours of receiving the DO report under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES section. During the designated operator inspection,the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing inspections at least once every 30 days at this facility and are including all of the required information on the reports.
