San Joaquin County April 17, 1997 <br /> Ms.Mary Meays -2- 953-7001 <br /> permit, pressure grouting the wells in accordance with the guidelines set forth in Section 6, <br /> of the Well Standards established by San Joaquin County,Public Health Services and <br /> reporting the results of the decommissioning to the SJCHD. <br /> Prior to decommissioning, the total well depth and the depth to the water table were <br /> measured in each well. The christy boxes were then removed. Monitoring well MW-4D was <br /> partially obstructed by a broken well cap which was removed prior to grouting.The grout <br /> mixture was mechanically mixed at a ratio of approximately 5 gallons of water per 94 lb. <br /> sack of Type I/II cement. The wells were then pressure grouted through a tremmie pipe <br /> from the bottom of the well to the surface. Grout was allowed to spill over into the christy <br /> box excavation. At locations within pave areas the upper 4 to 6 inches was backfilled with <br /> concrete to be flush with the surrounding pavement. The grouting procedure was observed <br /> by Mary Meays from the San Joaquin County, Public Health Services, Environmental Health <br /> Division. The well locations are shown on Figure 1.
