The <br /> wells will be grouted using tremie pipe from the bottom of the well to the surface with Portland <br /> cement slurry. After the cement slurry has been placed in the wells, the well heads wilLber-----'/0 )-r7 ;,-I <br /> pressurized to 25 pounds per square inch gauge (psig) for a minimum of five-rni`mutes. Following <br /> pressure grouting activities, the top fi\efeet of each well will be ov rdrilled and backfilled with <br /> the Portland cement slurry to within aof ground surface. Eac boreh le will be finished <br /> with a concrete cap dyed to match theunding surfaces. <br /> Previously Contaminated Wells <br /> Apex proposes to destroy over drilling to total depth two monitoring/vapor extraction wells <br /> (MW-'� EW-4 and te <br /> 8/VEW-3), two vapor extraction wells (V>jW=land V,1 W-2), onerecovery well (RNY0) five groundwater monitoring wells (MW-3, Molt, MW-11, <br /> MW'f5, and MW�`lg) associated with the subject site (Figure 2). The wells will be over-drilled <br /> to total depth with an auger equal to or greater than the original boring diameter. Following the <br /> over-drilling activities, the resulting borehole will be grouted with a Portland cement slurry using <br /> tremie pipe from the bottom of the well to the surface through the center of the hollow stem <br /> auger flights. The boreholes will be finished with a concrete cap dyed to match the surrounding <br /> surface. Underground remediation piping connecting the vapor extraction wells, recovery well, <br /> and ozone sparging wells will be removed if possible or will be grouted in place using a Portland <br /> cement slurry. <br /> All drill cuttings will be stockpiled on site and sampled for disposal purposes. The well <br /> destructions will be permitted with the SJCEHD and encroachment permits will be obtained for <br /> destruction of off-site wells. <br /> SCHEDULE <br /> Upon approval of this workplan by the SJCEHD, work will be scheduled for the destruction of <br /> all wells. Upon completion of destruction activities, a well destruction results report will be <br /> prepared and submitted to regulatory agencies. <br /> SITE SPECIFIC HEALTH AND SAFETY PLAN <br /> A site specific Health and Safety Plan(HASP) will be prepared, uploaded to geotracker, and will <br /> be retained'on site during all field activities. All work will be conducted according to the HASP. <br /> The HASP will contain information on the properties of the hazardous materials known to be on <br /> the site. This information is equivalent to that contained within Material Safety Data Sheets. <br />
