Limited Subsurface Site Investigation Results Report
Moore Petroleum Bulk Plant,5491 F Street,Banta,San Joaquin County,California
Page 3
July 25 - 26, 2000 - Apex personnel supervised the installation of three groundwater monitoring
r✓ wells (MW-10, MW-11, and MW-12). The results of all soil and water sampling were
documented in Results Report for Limited Subsurface Investigation and Third Quarter 2000
Groundwater Monitoring Report, dated August 25, 2000.
November 17, 2000 - Apex submitted Workplan for Interim Remediation &fourth Quarter 2000
Groundwater Monitoring Report, proposing the installation of three recovery wells and one
vapor extraction well, using the three recovery wells for a pumping test. SJCEHD responded and
only wanted one recovery well installed for the pumping test and depending on the results, more
recovery wells could be installed.
5
January 8, 2001 - Apex supervised the installation of one vapor extraction well (VEW-2) and one
groundwater recovery well (RW-2). Results of the well installation are documented in Results
Report for Limited Subsurface Investigation and First Quarter 2001 Groundwater Monitoring
Report, dated March 9, 2001.
March 2001 - Apex performed a pumping test utilizing well RW-2. The results of the pumping
test were reported in Apex's Second Quarter 2001 Monitoring Report and Interim Remediation
Plan, dated June 1, 2001.
August 24, 2001 - Apex over-drilled and reconstructed well MW-7 as a vapor extraction well.
On September 12, 2001, Apex personnel installed the remediation piping connecting wells MW-
7, VEW-4, VEW-2, and VEW-3 to the remediation compound.
June 2002 - Construction and assembly of the vapor extraction system was completed. The
official start-up of the system was on June 17, 2002 as reported in the Source Test Results
Report, dated July 31, 2002, and Remediation Status Report, dated October 4, 2002.
September 26, 2003 - Apex submitted a workplan that recommended in-situ chemical oxidation
by ozone sparging to remove the total petroleum hydrocarbons as diesel (TPHd)/motor oil (mo)
TPHmo mass from the soil and groundwater.
November 10, 2003 - An addendum to the workplan was requested in a letter by the SJCEHD to
increase the scope of work to further delineate the hydrocarbon plume by installing additional
monitoring wells. Technical data on the ozone system and the spacing of the sparge points was
also requested. On January 14, 2004, Apex submitted Addendum to Workplan for Ozone
Sparging.
January 14, 2004 - Apex submitted Workplan for a Limited Subsurface Site Investigation,
proposing the installation of five additional groundwater monitoring wells.
May 12, 2004 - The SJCEHD responded to the Addendum to Workplan for Ozone Sparging, by
approving the proposed ozone sparge wells and requested an addendum to the workplan,
proposing an additional cone penetration testing (CPT) boring and relocation of the proposed
