First Quarter 2005 Groundwater Monitoring and Remediation Status Report <br /> Moore Petroleum Bulk Plant,5491 F Street,Banta,California <br /> / Page 4 <br /> November 10, 2003 -An addendum to the workplan was requested in a letter by the SJCEHD to <br /> increase the scope of work to further delineate the hydrocarbon plume by installing additional <br /> monitoring wells. Technical data on the ozone system and the spacing of the sparge points was <br /> also requested. On January 14, 2004, Apex submitted a report titled,Addendum to Workplan for <br /> Ozone Sparging. <br /> January 14, 2004 - Apex submitted a workplan entitled, Workplan for a Limited Subsurface Site <br /> Investigation, proposing the installation of five additional groundwater monitoring wells. <br /> May 12, 2004 - The SJCEHD responded to the aforementioned Addendum to Workplan for <br /> Ozone Sparging, by approving the proposed ozone sparge wells and requesting application for <br /> drilling permits. The SJCEHD requested an addendum to the workplan,proposing an additional <br /> cone penetration testing(CPT) boring and relocation of the proposed groundwater monitoring <br /> well MW-17. <br /> August 16, 2004 - The SJCEHD directed in a letter that an addendum be made to the Workplan <br /> for a Limited Subsurface Site Investigation, dated January 14, 2004, in order to define the vertical <br /> extent of contamination. Apex submitted the report titled, Workplan Addendum Letter for a <br /> Limited Subsurface Investigation, dated June 2, 2004. <br /> September 2004 - Apex installed and tested an SVE system with activated carbon adsorption as <br /> the control device. In addition, Apex installed an ozone generator system for eventual ozone <br /> sparging. Results from the SVE startup were reported to San Joaquin Valley Air Pollution <br /> Control District (SJVAPCD) in a letter dated November 11, 2005, entitled, Source Test Report. <br /> December 6-8, 2004 -Apex supervised the drilling of nine ozone sparge wells (OS-2 through <br /> OS-10).
