Location of first aid kit: On drilling rig <br /> Nearest emergency medical facility: Tracy Community Memorial Hospital (835-1500) <br /> Directions from work site: Proceed south on Lammers Road and proceed approximately one half mile <br /> to Eleventh Street; turn left (east) on Eleventh Street and proceed approximately two miles to Tracy <br /> Boulevard; turn left (north) on Tracy Boulevard. and proceed 3.5 blocks to Hospital (on right @ 1420 <br /> Tracy Boulevard). <br /> Attach a map which shows the route from the work site to the nearest emergency medical facility, <br /> E7Check when done. <br /> KEY SAFETY PERSONNEL AND RESPONSIBILITIES <br /> All personnel working at the job site are responsible for project safety. The operational and health and safety <br /> responsibilities of pertinent personnel are identified below. <br /> PROJECT MANAGER: Russell Juncal <br /> The Project Manager is responsible for the provisions and submittal of this SSP to the Site Safety Officer <br /> P P Y <br /> (SSO) and for advising the SSO on health and safety matters. She/He has the authority to provide for the <br /> auditing of compliance with the provisions of this SSP,suspend or modify work practices, and to recommend <br /> disciplinary action for individuals whose conduct does not meet the provisions presented in this SSP. The <br /> Project Manager is also responsible for ensuring that Medical Surveillance Exams and Training Programs are <br /> current for all personnel working on-site. <br /> SITE SAFETY OFFICER: Russell Juncal <br /> The SSO is responsible for the dissemination of the information contained in this SSP to all personnel working <br /> at the job site,and to the responsible representative(s) of each subcontractor firm working at the job site. The <br /> SSO is responsible for ensuring the followingitems are adequately addressed: Safety Supplies and Equipment <br /> Inventory;Accident/Incident Reporting Procedures;Decontamination/Contamination Reduction Procedures; <br /> General Safe Work Practices. <br /> The SSO has the authority to suspend work anytime she/he determines the safety provisions set-forth in this <br /> SSP are inadequate to ensure worker safety. The SSO shall also inform the Project Manager of individuals <br /> whose conduct does not meet the safety provisions of this SSP. The Site Safety Officer reports to the Project <br /> Manager.
