FACILITY BACKGROUND INFORMATION <br /> Job Name: Souza/Nicolaw <br /> Job Location: Byron and Lammers Roads, Tracy,CA <br /> Client Name: Patrick Riddle <br /> Project Number: 016 Date: November 8, 1994 <br /> General Work Description: Installation of six monitoring wells <br /> SSP Prepared by: Russell W.Juncal <br /> On-site Safety Officer: Russell W.Juncal <br /> Project Safety Officer: Russell W.Juncal <br /> This site safety and health plan (SSP) must be kept on-site and accessible to all on-site <br /> employees during all phases of site operations. The SSP must be reviewed and signed by <br /> all on-site employees,subcontractors,and visitors prior to entering the site. The provisions <br /> set forth in this SSP shall apply to all employees and any subcontractors working at the <br /> job site. All personnel working at the job site must read this SSP, and sign the attached <br /> Compliance Agreement before entering the work area. Field personnel may deviate from <br /> the safety provisions set forth in this SSP, but only to upgrade or increase the safety <br /> requirements. Site personnel may suspend work if unsafe working conditions are identified <br /> or observed,or unauthorized modifications to the safety provisions set-forth in this SSP are <br /> made. If changes in site or working conditions require changes in safety procedures, <br /> appropriate amendments to this SSP will be provided by the project manager or supervisor, <br /> with the written approval of the branch safety officer or the Health and Safety Director. <br /> WORKPLAN <br /> Task 1. Drill six soil borings and install monitoring wells, if feasible, to approximate <br /> depths of 10 feet below ground surface. Collect and describe soil samples at 5 foot <br /> intervals. <br /> Task 2.
