S <br /> Blatt Commercial Brokerage 3 <br /> April 5,- 1990 <br /> 202/029000-080 <br /> The plan will address any potential chemical and physical exposures that Groundwater <br /> Technology personnel and any subcontractors may encounter while working at the site. <br /> A copy of this document will be reviewed by all personnel involved with the <br /> installation of the monitoring well, decontamination of equipment, or handling of <br /> potentially contaminated samples. <br /> 2.0 MONITORING WELL INSTALLATION <br /> Groundwater Technology personnel will acquire the proper permits and supervise <br /> the installation of one 2-inch-diameter monitoring well in the location noted on the site <br /> plan (Figure 1). The well will be installed with a truck-mounted drill rig fitted with <br /> hollow-stem augers that are approximately 8 inches outside diameter. The well will be <br /> constructed with schedule 40 PVC casing and 0.020-inch slotted screen, with a threaded <br /> cap at the bottom. The screened interval will extend 10 feet below and, if possible, 5 <br /> feet above the water level, depending upon what depth groundwater is encountered. The <br /> annulus between the borehole wall and the well casing will be filled with Lonestar No. <br /> 2/12 sand until the level of the sand is one foot above the top of the screened interval. <br /> The annulus will then be filled with bentonite pellets until the level of bentonite is a <br /> minimum of one foot above the level of sand. The remaining length of annulus will be <br /> sealed with grout to the surface. The grout will consist of a mixture of sand, cement, and <br /> powdered bentonite, with a maximum ratio of two parts sand to one part cement by BOG f-- <br /> weight. The well will be finished at grade with a protective road box. <br /> A Groundwater Technology geologist will collect and describe the soil samples to <br /> prepare a log that will include sample locations, subsurface geology, and well construc- <br /> tion. Soil samples will be collected with a split-spoon sampler driven 18 inches ahead of <br /> the auger at 5-foot intervals, beginning at 5 feet below grade.
