The consultant must employ qualified persons and/or sub contractors to perform the work described above, including licensed professionals and other technical professionals, and possess all necessary licenses to operate within the State. 3. The consultant must carry out the described work in a timely, professional, safe and legal manner and maintain necessary records and other safeguards to ensure that reported sample results are accurate. All work must be performed and reported in a manner acceptable to the DEH. 4. The consultant must provide at least 5 references for whom the consultant has performed similar work, in the last 5 years. This must be provided with the bid package, and should include names and phone numbers of the person to verify the work reference. 5. The consultant must maintain at least 1 million dollars in General Liability Insurance, and in addition must provide and maintain at least 1 million dollars in Professional Liability Insurance (E&O), including accidental pollution coverage. Proof of insurance must accompany the bid, and a telephone number and a contact person at the insurance company must be included. No exceptions to the E&O insurance will be considered. 6. Work shall be performed in a manner which will not interfere with the normal operations of the DeBoer Truck Lines unless absolutely necessary. 7. Consultants shall provide a copy of their standard services agreement and schedule of charges with any bid submitted. The schedule provided in this bid package must be filled out. Additional information can be provided by the consultant if necessary. 8. Work shall begin within 5 days of execution of the standard services agreement unless weather or other conditions delay the project. 9. Consultant understands that DeBoer Truck Lines reserves the right to reject any and all bids without cause or notice. DeBoer Truck Lines retains the right to solicit and accept additional bids as necessary in it's judgment. All bids are due June 10, 1993. 10. Consultant understands that DeBoer Truck Lines intends to seek reimbursement for the subject work from the UST Cleanup Fund (SB2004) and agrees to respond to any reasonable inquiry from the administering agency regarding any claim submitted to DeBoer Truck Lines to that agency. 11. All bid packages must include- both the consultants proposal for work to be performed as well as the cost for the proposed work.
