PUBLIC HEALTH SERVICES ,o¢ .FA
SAN JOAQUIN COUNTY ¢ i
ENVIRONMENTAL HEALTH DIVISION
Karen Furst, M.D., M.P.H., Health Officer
304 East Weber Avenue, Third Floor • Stockton, CA 95202
209/468-3420
To: Keith Sheets
From: Dot Lofstrom
Re: Well/Boring Permit# SR0026833
Date: September 17, 2001
On July 23, 2001, San Joaquin County Public Health Services, Environmental Health Division
(PHS/EHD)issued well permit#SR 0026833 to Gregg In Situ drilling company to install six
cone penetrometer test borings on a public right of way administered by the City of Stockton.
Subsequently, a grab groundwater sample from CTP-16 had a detected concentration of 360
micrograms per liter of 1,2-dichloroethylene. Therefore, in a meeting between you, Richard Fish
of Del Monte, and myself representing PHS/EHD, we agreed that additional CPT borings would
be installed to complete the investigation. According to Kelly Chan of CH2MHill,the City of
Stockton is extending the encroachment permit previously issued for this work, rather than
issuing a new encroachment permit. Since the City of Stockton is not requiring Gregg In Situ to
submit a new application for encroachment on public right-of-way, PHS/EHD will not require a
new well permit. Therefore, the additional work may be carried out under existing well permit
#SR0026833.
`II
I
I
.
