4.4.1.1 Sample Labels <br /> All samples will be labeled with information identifying the sample location, date and <br /> time of sample collection, sampler, and other relevant data, as appropriate. All sam- <br /> ples will be assigned a unique identifying number that will be written on the sample <br /> label and affixed as a separate printed label. <br /> All labels will be filled out using indelible ;nk and affixed to the sample containers via <br /> the adhesive backing, if appropriate. 'Nide, dear plastic tape will then be placed over <br /> the lobel to protect it and to further secure the label to the sample container. An <br /> g Figure on <br /> example of the sample label to be used is given Fure 4-2. <br /> 4.4.1.2 Chairs-of-Custody Record <br /> A Chain-of-Custody Record will be filled out for each sample shipment container. All <br /> samples within a container must be accounted for an the Chain-of-Custody Record. An <br /> example of the Chain-of-Custody Record is given on Figure 4-3. <br /> 4.4.13 Transfer of Custody and Shipment <br /> r Samples are accompanied by a Chain-of-Custody Record. When transferring samples, <br /> the individuals relinquishing and receiving them sign, date, and note the time on the <br /> record. This record documents sample custody transfer from the sampler, often <br /> through another person, to the analyst at the laboratory. <br /> Samples are packaged properly for shipment and dispatched to the appropriate labara- <br /> tory for analysis,with a separate Chain-of-Custody Record accompanying each shipping <br /> ` container. Shipping containers will be sealed with custody seals for shipment to the <br /> laboratory. The method of shipment,courier name(s), and other pertinent information 3 <br /> are entered in the Remarks Section of the Chain-of-Custody Record. An example of { <br /> `m the custody seal is given on Figure 4-2.. All shipments are accompanied by the Chain- <br /> of-Custody Record identifying its contents. <br /> if sent by mail,the package is registered with return receipt requested. If sent by colrt- <br /> s-= mon carrier, a bill of lading is used.
