The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control <br /> District for applicable permits and requirements. <br /> 2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br /> commencing work. All new addendums supersede the previous ones. <br /> 3. All components must be certified by an independent testing organization and labeled as <br /> required by the independent testing organization. <br /> 4. At least one person who holds the International Code Council(ICC) "UST Service <br /> Technician"certification and manufacturers' certificate of training for all components <br /> installed or tested must be onsite at all times when work is performed. <br /> 5. Each person installing or testing any component must hold a certificate of training from <br /> the manufacturer of that component. <br /> 6. The"Certificate of Compliance for Underground Storage Tank Installation Form C"shall <br /> be submitted by the owner or the owners' agent[pursuant to California Code of <br /> Regulations(CCR)2635 (d)]before this replacement will be considered complete by <br /> SJCEHD. <br /> 7. If concrete is broken over the tank, a third party tank integrity test must be performed by <br /> a certified tank tester once concrete is poured and test results submitted to SJCEHD <br /> within 7 days of completion of work. <br /> 8. Contact Toua Yang at(209)468-3459 to arrange the following inspection appointments. <br /> a. EHD must verify the overfill prevention system is in compliance with CCR 2635 <br /> (b)(2).Note: Your company may choose to have EHD onsite during the overfill <br /> prevention system installation so that portions of the system do not have to be <br /> disassembled/reassembled. <br /> b. The spill bucket must be tested to ensure it holds a minimum of 5-gallons and <br /> pass a one hour lake test or equivalent. <br /> c. If concrete is broken over the tank, corrosion protection of the risers must <br /> be verified prior to backfilling around the spill buckets. <br />
