Site Code: 2005 <br /> 848 W Fremont St. <br /> Stockton CA 95203 <br /> In a letter dated January 28, 2003, San Joaquin County Environmental Health <br /> Department (EHD) directed you to submit a work plan to destroy the existing <br /> monitoring wells at the above-referenced site to EHD by March 1 , 2003. EHD <br /> has not received this work plan to date. <br /> Please be aware that your site has not been closed. Destruction of all existing <br /> monitoring wells by a C-57 licensed contractor under EHD inspection is required <br /> before a "no further action" status can be granted. San Joaquin County Well <br /> Standards, Section 6.1 states that a well must be destroyed if it "no longer <br /> functions as originally designed." Submit a work plan to destroy the remaining <br /> monitoring wells at the site to EHD by May 23, 2003. <br /> If you wish to retain the wells, you will be required to submit a request in writing <br /> and how you plan to use the wells, and an annual report on how the wells were <br /> used, and pay for EHD staff report review time. Current hourly rate for EHD staff <br /> time is $89 per hour.
