i <br /> 12.0 TRAINING AND MEDICAL SURVEILLANCE <br /> i� <br /> All Dames & Moore site personnel will have met the requirements of 29 CFR <br /> 1910.120 (e) , including: <br /> o Forty hours or initial off-site 'training or its recognized <br /> equivalent; <br /> o Eight hours of annual refresher training for all personnel; <br /> s <br /> i <br /> o Eight hours of supervisor training for personnel serving as Site <br /> Safety Officers; <br /> o Three days of work activity under the!isupervision of a trained and <br /> experienced supervisor. <br /> All Dames & Moore site personnel are participating in medical surveillance <br /> programs that meet the requirements of 29 CFR 1910I1120(f) . The PM will maintain <br /> current copies of training certificates and statements of medical program <br /> participation for all site personnel. <br /> LIn addition, all Dames & Moore site personnel will review this HSP and sign <br /> a copy of the safety plan compliance agreement, which is found in the <br /> Attachments.
