• Reporting to the PM or SSHO a personal condition that they reasonably believe <br /> could affect their safety and/or the safety of coworkers (i.e., fatigue, drowsiness, <br /> severe illness, impairment by prescription medications, influence by drugs and <br /> # alcohol, emotional distress or other condition) , <br /> Ensuring that no work tasks are performed that deviate from the HASP, and/or the <br /> initial instructions of the Field Supervisor, or SSHO, without the expressed <br /> authorization and additional instruction from the Field Supervisor and/or SSHO <br /> j 3.2.5 Visitors <br /> Visitors, including State, and/or local entities may visit the Site, but will be responsible for the <br /> following: <br /> 1 • Receiving site hazard and safety instructions from the SSHO <br /> • Reviewing and complying with the essential elements of the HASP <br /> • Using their own, or provided,personal protective equipment to enter regulated <br /> work areas when such controls are required for entry as per the HASP <br /> • Reporting any observed unsafe act and/or condition at, or affecting, the work site <br /> i to the SSHO <br /> f � <br /> Visitors are prohibited from entering controlled zones without the use of specified PPE and <br /> appropriate training and medical monitoring information. <br /> Table 3-1 <br /> KEY PERSONNEL <br /> Key Personnel Responsibilities Phone Number <br /> ?
