Comments: 1.)A/C T14 CCR§ 18459.3- Facility was missing manifests for the following dates: 12/4/2015, 10/7/2015, 9/16/2015, 9/1/2015, 7/29/2015. a.) Facility shall maintain a copy of the completed manifest form at their place of business. Manifest forms shall be retained for 3 years and be available for review during inspection. b.) Ensure subsequent manifests are retained for 3 years. 2.) A/C T14 CCR§ 18462 -The following CTL forms had an incorrect facility TPID listed: 4596025-A, 4596029- C, 4596024-B, 4796504-A, 4596030-C, 4596029-A. a.)Waste tire manifests shall be filled out completely and accurately. b.) Ensure subsequent manifests are filled out correctly before signing. Notes: 1.) TPID certificate is posted in the main office. 2.) Permission to conduct inspection was granted by Mark Valadez (EHS Regional Manager). 3.) This facility use "Pape Material Handling Inc."to haul their waste tires. 4.) Retain all waste tire disposal receipts, logs, and manifests for 3 years. 5.) No waste tires are stored on site. The waste tire is removed by the hauler once service is complete. L L L;
