Site-Specific HASP will be reviewed by field staff and contractors before beginning field <br /> operations at the site. <br /> 3.2 Permitting <br /> Cardno ATC will obtain any necessary encroachment permits from the City of Stockton and Port <br /> of Stockton and well destruction permits for on-site and off-site well destructions from the San <br /> Joaquin County Department of Environmental Health (SCDEH) prior to beginning work. Well <br /> destruction activities will be observed and approved by a SCDEH representative, and post- <br /> destruction surface finishing of off-site wells will be approved by a City of Stockton and/or Port <br /> of Stockton representative if required. <br /> 3.3 Subsurface Utility Clearance <br /> Prior to field activities, Cardno ATC will mark all proposed drilling locations in accordance with <br /> the Underground Service Alert (USA) guidelines, and notify USA North of upcoming subsurface <br /> activities so existing underground utilities in the area of proposed work can be located and <br /> avoided. Cardno ATC will also contract a private utility locator to confirm the locations of the <br /> underground utilities in the vicinity of the work and attempt to identify any other subsurface <br /> conflicts with the proposed work locations. <br /> 3.4 Well Destruction Activities <br /> Destruction of the 19 monitoring wells, 6 piezometers, and 9 injection wells at the site will be <br /> completed by over-drilling each well with augers of the same diameter or larger as were used to <br /> install each well. Over-drilling will employ a centering rod to keep the auger on the same <br /> vertical path as the installed well, and will be completed to the same depth as the well being <br /> removed. Upon removal of all installed well materials, each boring will be grouted to within a <br /> foot of the ground surface, and the open boring backfilled with sand or bentonite to just below <br /> ground surface, per SCDEH guidelines. The surface will be finished with cement or asphalt to <br /> match the surrounding grade at the request of the City or Port of Stockton. <br /> 3.5 Waste Disposal <br /> Investigation derived waste (IDW) including soil cuttings, groundwater, and decontamination <br /> water generated during well destruction activities will be contained in DOT approved 55-gallon <br /> drums or a roll-off bin and temporarily stored on site. The IDW will be profiled and scheduled for <br /> removal and disposal by a licensed waste disposal contractor as soon as possible. <br /> 4.0 LIMITATIONS AND CERTIFICATIONS <br /> This report was prepared in accordance with the scope of work outlined in Cardno ATC's <br /> contract and with generally accepted professional engineering and environmental consulting <br /> practices existing at the time this report was prepared and applicable to the location of the site. <br /> It was prepared for the exclusive use of Philips 66 Company, for the express purpose stated <br /> above. Any reuse of this report for a different purpose or by others not identified above shall be <br /> at the user's sole risk without liability to Cardno ATC. To the extent that this report is based on <br /> information provided to Cardno ATC by third parties, Cardno ATC may have made efforts to <br /> verify this third party information, but Cardno ATC cannot guarantee the completeness or <br /> accuracy of this information.
