PREPARED BY: Wesley Snyder <br /> WORK REQUEST FORM <br /> SCOPE OF WORK: Oxygen Injection , <br /> _ 1) Sign in at the Nustar Energy Facility(2941 Navy Drive,Stockton)-Check-in with Terminal Representative <br /> Work with terminal representative in filling out the days work permit and on getting access to the site. <br /> Required PPE includes Nomex coveralls,hard hat and safety glasses at all times. <br /> Arrive at site- Review(1) HASP (2)work order,then call Ed 384-0773 or Sean 916-281-8975 <br /> 2) Collect Pre-Injection DO and Initial DTW Readings (See DTW& DO FORM) <br /> 3) Based on Instruction from Ed or Sean follow the days plan for which wells <br /> (IP-10 IP-11 IP-14 IP-15 IP-16 IP-17 MW-22)to inject into. <br /> Inject into two wells simultaneously at a maximum.
