Specific violations and required corrective actions are listed below. Failure <br /> to correct the identified violations within the scheduled time period will result in PHS- <br /> EHD citing you for continuing/additional violations. <br /> I. REPORT OF VIOLATIONS <br /> 1. Title 22, California Code of Regulations, Division 4.5, Section 66268.7 (a)(6): <br /> A Land Disposal Restriction (LDR) notification was lacking from manifest <br /> numbers 90900658, 90900659, and 90900693. <br /> 2. Title 22, California Code of Regulations, Division 4.5, Section 66268.7 (a)(6): <br /> A LDR notification and certification were lacking from manifest numbers <br /> 90900694 and 90900701. <br /> II. SCHEDULE OF CORRECTIONS <br /> Violation #1: Effective immediately, all Land Disposal Restriction (LDR) notifications <br /> shall be kept with appropriate manifest. <br /> Violation #2: Effective immediately, all LDR notifications and certifications shall be <br /> kept with appropriate manifest. <br /> A Division of San Joaquin County Health Care Services <br />
