� � s <br /> 21 Permits - Prior to commencement of drilling, an approved well permit would be obtained <br /> from the San Joaquin County Public Health Services Environmental Health Division <br /> t (SJPHS). SJPHS will be notified 48 hours before any drilling or sampling begins <br /> 22 Underground Utilities - Underground Service Alert(USA)will be notified 48 hours before <br /> any subsurface work begins. <br /> 23 Site Safety Plan - Work would be performed under the guidelines presented in the BSK <br />' prepared Health and Safety Plan presented in Appendix A. <br /> 3.0 GROUNDWATER MONITORING WELLS <br /> INSTALLATION AND DEVELOPMENT <br /> A BSK & Associates geologist under direct supervision of a Registered Civil Engineer will be <br /> present during drilling, soil sampling and groundwater sampling. <br /> rThe locations of the proposed groundwater monironng wells (MW-5 and MW-6) are presented <br /> on Figure 2, Site Plan. Drilling will be accomplished with a Mobile Drill B-53 truck-mounted <br /> ng with 8-inch hollow stem auger Soil samples will be collected at IQ-foot intervals by driving <br /> a 14 I D. sampler 18 inches into relatively undisturbed soil The soil samples will be collected <br /> for the purpose of soil classification. <br /> Augers and dnii rods will be cleaned with a high-pressure, high-temperature cleaner before, <br /> between and after each bore hole. The equipment was washed in a polyethylene-lined <br /> containment basin. The rinsate from the auger wash will be placed into 55-gallon DOT 17 E/H <br /> drums and labeled. Soil cuttings will be placed upon and covered with 6-mil polyethylene <br /> sheeting <br /> Well construction materials would consist of 2-inch diameter schedule 40 PVC casing screen with <br /> flush threaded,joints Prior to installation, the PVC casing and screen would be washed with a <br />' high-pressure, high-temperature cleaner. The annular space around the slotted interval will be <br /> back-filled with washed, kiln-dned, graded #2112 sand The sand will be placed through the <br /> hollow stem auger to a level two feet above the slotted interval A three-foot layer of 1/4-inch <br /> tbentonite pellets will be placed above the sand pack and hydrated with potable water Neat <br /> cement will be placed above the bentonite pellet seal by the tremae method A water-tight, <br /> traffic-rated vault will be installed at surface grade at each well location <br /> The wells will be developed by surging, bailing and pumping until the water reaches a degree <br /> of clarity. The well construction details are presented on Figure 3, Monitoring Well Details. <br /> 4.0 ELEVATION SURVEY <br /> The relative elevations of the top of casing of the two additional wells and one existing well <br /> would be determined using a surveyor's level.
