1 <br /> Employees are not to stand upon or work from containers or debris piles at any time. <br /> U.S Department of Transportation (DOT) specified salvage drums or containers and suitable <br /> quantities of proper absorbent shall be kept available and utilized in areas where spills, leaks <br /> or ruptures may occur Containers that cannot be moved without rupture, leakage or spillage <br /> shall be emptied into a sound container using a device classified for the material being <br /> transferred <br /> ' 4.2.4 Excavation Safety <br /> The following rules will be enforced for all excavations within the work site <br /> • The location of any underground utilities willbe identified prior to commencing <br /> excavation activities and workers will be informed of these locations <br /> • The FC shall inspect excavations for signs of weakness or structural defects a <br /> the start of each shift or when environmental or work conditions change <br /> significantly <br /> • Personnel will be trained m the specific hazards associated with excavation and <br /> trenching prior to entering the work area <br /> n r an excavation or trench <br /> No one shall be permitted toe to deeper than 5 feet <br /> without first contacting the HS Representative for an addendum to this plan <br /> ' Barricades or barriers will be placed around the excavation site to prevent <br /> unauthorized entry and to warn equipment operators <br /> ' <br /> Heavy equipment not being used in excavation operations shall be placed a <br /> • �'Y q uiP <br /> sufficient distance from the excavation pit so that their weight does not weaken <br /> the excavation walls. <br /> • The spoil shall be placed a sufficient distance (two feet minimum, four feet <br /> recommend) from the excavation so as not to create stress on the excavation <br /> walls <br /> ' Blades and buckets on the front ends of heavy equipment shall be lowered <br /> during transport and whenever the operator leaves the machine <br /> ' Construction equipment shall be given the right-of-way during field activities. <br /> • Heavy equipment shall have a reverse signal alarm that operates automatically <br /> MZ/11-29-94/RHS/HASP/94-0037.JAF 4-4 <br />
