The vaiero operator on duty spotted product <br />coming out of the truck dome and got the attention of the driver and hit the emergency <br />shut off, stopping flow to the truck. Estimated approximately 100 gallons of clear diesel <br />spilled from the top of truck, into the contaiment area of the truckrack and into the <br />closed sump drain system and tank. <br />Operator immediately shutdown all truck rack operations, and informed all drivers to <br />leave vehicles alone, no starting of engines or moving of trucks. Operators called Shawn <br />Mendoza,Area Mgr. Shawn Mendoza arrived at terminal, met with operators, evaluated <br />hazards, and after checking the truck rack with lel meter for vapors, other trucks in the <br />Immediate Actions Taken: <br />rack were removed. The truck involved was cleaned and cleanup operations were begun. <br />The driver was questioned and information is still being investigated. Notifications were <br />made to Rich Brandes, HSE Director and Mike Peterson, General Manager. National <br />response center was notified, and report # 804486 was recorded. More notifications to <br />local agencies will take place tomorrow.
