Before Mr. Davis will be allowed to load at the Stockton Terminal <br />again, we will need to have responses from Bartlett Petroleum on the <br />following items: <br />Driver Training: <br />The driver was unaware of retain in the truck and entered the wrong <br />presets for loading. <br />The driver was not at the meter which is a standard operating procedure of <br />this facility, ( written policy included). He was on the opposite side of <br />the meter and loading arm, talking with a driver in another lane. If he <br />had been at the loading side of the truck and at the meter, he would have <br />been able to stop the overflow sooner. <br />Mr. Davis will have to schedule a loading appointment, to be evaluated on <br />his knowledge of our loading equipment and procedures prior to having full <br />access to loading privileges at this terminal again. <br />Bartlett Equipment: <br />We would like to have third party documentation that the high level <br />sensors in this semi have been checked since the spill and are <br />operational. We would appreciate any other preventative maintenance <br />documentation you can provide for the trucks that load at this terminal. <br />Bartlett Investigation: <br />We would like to know that Bartlett also has investigated this incident <br />and will work with us to avoid a repeat. <br />Valero LP is committed to maintaining the highest safety and environmental <br />standards at it's facilities. We feel we have fairly investigated this <br />incident, and believe it was caused by a combination of driver error and <br />equipment failure ( truck high level sensors). <br />I do appreciate the time and honesty you have given our concerns on <br />this issue so far, Mr. Gomez, and I will be glad to assist with putting <br />this incident behind us and moving on. <br />Mr. Davis will remain unable to load at this facility until we have <br />received a written response to these requests that is acceptable. <br />Sincerely <br />Shawn Mendoza <br />Area Manager / West Region <br />Valero LP <br />CC: Mr. Rich Brandes /HSE Director - West Region <br />
