S T SERVICES INC <br /> KEY STOP AGREEMENT <br /> It is requested that keys for keystop operations, as set out in the keystop <br /> agreement between S T Services Inc. and , be issued to <br /> as employee and representative of <br /> By this request, we do hereby acknowledge that our employee is bound by the <br /> terms of the subsequent agreement on this page to which his signature is <br /> affixed. We agree to notify S T SERVICES Terminal Personnel immediately if <br /> the Employee mentioned herein is terminated from our employment, transfered, <br /> or for any other reason has no immediate need for these keys in the performance <br /> of keystop loading, and to cause said keys to be returned to S T SERVICES INC. <br /> Name of Company REPRESENTITIVE <br /> TITLE DATE ISSUED <br /> IEY RECEIPT AND REPRESENTATIVE ASI <br /> I , employee and representative of <br /> have received key numbers frau S T SERVICES INC. for Key- <br /> Stop loading at the STOCK'I'ON, CA, Terminal. It is agreeded that the keys <br /> issued to me will be used only for withdrawal of petroleum products in accordance <br /> with this keystop loading agreement, and instructions mentioned herein. I will <br /> completely fill out the loading log sheet useing the totalizer numbers only. <br /> Record the opening meter reading in the apporate colum and CHECK THE COLSEING <br /> METER READING OF THE PREVIOUS LOAD. IF MY OPENING READING IS NOT THE SAME AS <br /> THE CLOSEING READING OF THE PREVIOUS LOAD, I WILL CALL ONE OF THE TE=IAL <br /> PERSONNEL AND REPORT IT. IF THERE IS A DIFFERANCE AND DID NOT REPORT IT , THE <br /> CARRIER WILL BE CHARGED WITH ANY MISSING PRODUCT. I will follow any instructions <br /> posted in the dog houses, and posted on the meters.I assume full responsibility <br /> for the custody of these keys and do hereby agree that no other person shall <br /> have access to therm, and in the event I am transfered, seperated from employment <br /> of my transporting employer, or for any reason whatsoever have no immediate <br /> need for these keys in the performance of key stop loading, I will promptly <br /> return these keys to the terminal. <br /> ANY DANAGE TO INSTALLED EQUIPMENT CAUSED BY ME WILL BE REPORTED TO TERMINAL PERSONNEL. <br /> I understand that access to the aforementioned terminal is discretionary with <br /> S T SERVICES INC. ,and that such grant may be revoked by S T SERVICES INC. at <br /> any time, <br /> I have read and fully understand this agreement, and acknowledge receipt of a <br /> copy of this agreement. <br /> TERMINAL MANAGER COMPANY REPRESENTED <br /> DRIVER REPRESENTATIVE <br /> DATE RETURNED DATE ISSUED �7 <br /> 7e <br /> z�mss= Z <br /> E d� <br /> �K <br />
