i <br /> Scope of Work: The purpose of project is to drill three soil borings aiJollect depth discrete groundwater <br /> samples. <br /> The site is a 305 by 130 foot triangle-shaped parcel with an active service station. <br /> Brown and Caldwell performed facility upgrade activities in 1995. In 1996, one groundwater monitoring well <br /> was installed and sampled until 1997. In 1997 the site received "No Further Action" status from CRWQCB- <br /> CVR. In 1997 Delta performed oversight during a facility upgrade. <br /> All work for this project will be scheduled during typical daylight hours. <br /> Site plan is in Attachment 1. <br /> Emergency Response: <br /> A. Injury or Illness: <br /> If an injury or illness occurs, take the following action: <br /> ♦ Determine if emergency response (fire/ambulance) staff is necessary. If so, dial 911 on cellphone or <br /> closest available phone, which is located at the patient reception/waiting areas in the ARCO#5504 station <br /> building. If it makes sense to take an individual to the hospital, use the directions on page 2: <br /> ♦ Get First Aid for the person immediately. Utilize first aid kit in site safety officer's vehicle. Also utilize <br /> the bloodbome pathogen kit. <br /> ♦ Notify the Site Health and Safety Officer, Todd Brown , immediately. The Site Health and Safety Officer <br /> is responsible for preparing and submitting the Injury/Illness Incident Report to the Health and Safety <br /> Director within 24 hours, as well as notifying the employee's supervisor and Principal-in-Charge.
