Quality Control Board concurred <br /> with the San Joaquin County.Environmental Health Department (EHD) determination of No Further <br /> Action Required (NFAR). concerning investigation and remediation of the contamination from the <br /> former underground storage tanks at the above-referenced site. Before a NFAR letter can be provided <br /> toyou,.the monitoring wells at the site must be properly destroyed.. <br /> Submit a work plan to.destroy the wells, an EHD.permit application, and$60 permit fee. Please note <br /> that monitoring wells in areas of contamination may only be destroyed by removing all of the well <br /> contents by drilling out the original borehole and filling it with grout. <br /> In addition to. the above, please provide EHD certification in writing that all record fee title owners <br /> have been notified of your request for closure. This is in accordance with Section 25297.15-of the <br /> California Health and Safety. Code. Submit a list of all record fee title owners and their mailing <br /> addresses..EHD.will send notification to each title owner of the pending closure letter...If you are the <br /> sole property owner, provide a statement to this effect to EHD. A form is enclosed for your <br /> convenience.
