Fremont St. <br /> Stockton, CA 95205 <br /> We have received formal comments from Ms. Elizabeth Thayer, of the Central Valley Regional Water <br /> Quality Control Board (CVRWQCB), regarding your recent request for closure evaluation. <br /> As we briefly mentioned in our July 28, 1992 letter, closure cannot be considered at this time. Many of <br /> the points discussed in Ms. Thayer's attached letter have been provided to you in previous PHS/EHD <br /> directives (see enclosures). <br /> It will be Nor-Cal's responsibility to submit by September 18, 1992, a work plan for the installation of at <br /> least 2 additional monitoring wells as well as the additional soil investigation. <br /> The second quarterly report for this year has not been submitted to this office. You have been instructed <br /> by written correspondence (August 26, 1991) and by telephone message (May 22, 1992) that until <br /> closure has been satisfactorily agreed to by both PHS/EHD and CVRWQCB, quarterly monitoring is <br /> required by law. [Section 2652(d), UST, CAC, Title 23 Subchapter 16] <br /> Quarterly data shall be submitted October 15, 1992 and is to include the survey of nearby wells. The <br /> "Problem Assessment Report" (PAR) is due December 15, 1992 and should include all investigative data <br /> compiled, quarterly monitoring conclusions, well survey data, as well as recommendations and <br /> conclusions for remediation of existing soil and/or groundwater contamination. <br /> We are enclosing the guidance documents for the preparation of the PAR. <br /> The workplan for field activities should be submitted to PHS/EHD, and any questions regarding the <br /> issuance of local permits in order to accomplish the PAR should be directed to Diane Hinson, <br /> Supervising RENS, PHS/EHD, at (209) 468-3452. <br /> Jogi Khanna, M.D., M.P.H. <br /> Health Offic r <br /> Laurie A.
