24 July 2009 <br /> AGE-NC Project No. 07-1524 <br /> Page 2 of.3 <br /> WELL DESTRUCTION PROCEDURES ' <br /> i <br /> On 04 June 2009, monitoring wells MW-1 through MW-3 were destroyed at,the site, under EHD <br /> oversight,utilizing a CME 75 truck mounted drill rig equipped with 8.25-inch diameter hollow stem <br /> augers.The drill rig and three-person crew were supplied by All Well Abandonment,of Placerville, <br /> California. <br /> Monitoring wells MW-1,'MW-2 and MW-3 were destroyed by over-drilling each well to its total <br /> depth with the 8.25-inch diameter hollow stem augers and a solid drive lead-bit. Well casings, <br /> screens and other materials associated with the wells were removed from the boreholes, and then <br /> each boring was grouted to near surface using a portland cement grout mixture.A ratio of six gallons <br /> water for every 94-pound sack of portland cement was utilized for preparation of the grout mixture. <br /> Auger returns generated by drilling were containerized in properly labeled Department of <br /> Transportation (DOT)-approved model 17H 55-gallon drums and were stor4on-site. <br /> NON-HAZARDOUS WASTE DISPOSAL <br /> On 30 June 2009,approximately 50 gallons of non-hazardous purge water/rinseate(generated from <br /> monitoring events and well destructions) and 1,000 pounds of soil cuttings and solid waste <br /> (generated from well destructions)containerized in.55-gallon drums were removed from the site by <br /> Advanced Chemical Transport,Inc. (ACT)of Merced,California.Non-hazardous liquid waste was <br /> transported and disposed of at Evergreen Oil, Inc., located in Newark, California. Non-hazardous . <br /> solid waste was transported and disposed of at US Ecology, located in Beatty, Nevada. t <br /> Non-hazardous waste manifests are included in Appendix C. <br /> Advanced GeoEnvlronmental,Inc. <br />
