Fremont Street <br /> Stockton CA 95205 <br /> San Joaquin County Environmental Health Department (SJC/EHD) notified you by <br /> correspondence dated April 25, 2002 that the "Work Plan for Lateral and Vertical Extent <br /> of Soil and Groundwater Contamination Addendum" dated May 31 , 2000 and submitted <br /> on your behalf by WHF Inc. had been conditionally approved . <br /> Please proceed with scheduling the fieldwork to implement the work plan per the <br /> SJC/EHD conditions . Specifically, work that needs to be completed at this site includes <br /> proposed soil borings for investigation of the vertical extent of the contamination in soil <br /> and groundwater, the installation of three shallow groundwater monitoring wells onsite <br /> and the completion of a well/receptor survey. Two cone penetrometer test borings that <br /> were advanced onsite in November 2001 failed to reach the depths proposed to <br /> complete vertical assessment at this site . <br /> A well permit application and required $89. 00 permit fee must be submitted to SJC/EHD. <br /> Forty-eight hours notice is required prior to beginning fieldwork. <br /> The deadline for completion of the fieldwork is May 30 , 2003 . A report of findings <br /> documenting the results of the fieldwork must be submitted to SJC/EHD within 60 days <br /> following completion of all fieldwork.
