The enclosed Well Permit Application Form should be completed and submitted with an$89 <br /> permit fee and a Site Specific Safety Plan. <br /> In order to become and remain eligible to receive reimbursement from the State Water Resources Control Board <br /> (SWRCB)Clean Up Fund(CUF),UST sites are required to be in compliance with directives and deadlines issued <br /> by PHS-EHD. Please ensure that you allow adequate time to have the original submittals(no faxed work plans <br /> accepted)to PHS-EHD by the date noted in this correspondence. <br /> Your site has been assigned to Michael Infurna,Senior REHS,and you should address all site mitigation and <br /> subsurface investigation correspondence to him.
