In the report,the notation"b" was added in the "well <br /> ID" column and referred to as post-derelgbrrM uaterlcza�" Were the wells re-developed on this date? <br /> On August 16, 1999,PHS-EHD directed you to delineate the lateral and vertical extent of the soil <br /> and groundwater contamination for this site. Your response to this directive was an incomplete <br /> investigation in December 1999,which failed to delineate the lateral,northwest extent of the soil <br /> contamination and groundwater plume. Data from the December 1999 investigation were included <br /> in the report of findings,titled"Site Investigation Report" and dated April 14,2000. Conclusions <br /> included in this report claimed that the "u7tiad ecte-r cf the par&=b)drorarbvs to the soil is delinaztErl in <br /> the vanity cf the nwbe n dispenser isbnd PHS-EHD disagrees with this conclusion. Monitoring well <br /> "S-1" represents the northwest area of petroleum hydrocarbons at this site. Samples collected from <br /> this well consistently have high levels of petroleum hydrocarbon contamination. Additionally, <br /> hydraulic push points "SB-4, 5 &6" placed during December 1999 identified soil and groundwater <br /> contamination at historical depths of concern. Thus,the northwest extent of the groundwater <br /> plume has not been defined at the site. <br /> PIS-EHD has been patient with your apparent reluctance in managing or proceeding with the <br /> minimum requirements of investigating or mitigating the release at this site. By not continuing to <br /> follow up on the incomplete investigation of December 1999,you have not complied with the <br /> requirements and directives issued by PIS-EHD. <br /> You are hereby directed to continue your subsurface investigation for this site by defining <br /> the lateral extent of the petroleum hydrocarbon plume in the groundwater and soil. By <br /> March 23, 2001 a work plan for further work to define the northwest extent of the <br /> groundwater plume is to be submitted to PHS-EHD. Offsite municipal/private access <br /> agreements,permits, and the PHS-EHD boring permit application and fee are to be <br /> included in this submittal. <br /> A Division of San Joaquin County Health Care Services <br />
