The following is an itemized list of permit by rule tiered permit violations that have <br /> not been addressed for PREMIER FINISHING as of June 27 , 2019 . <br /> Open violations from January 15 , 2019 inspection <br /> operator or by the generator of the hazardous waste , if the waste is received from off-site , that is used as the basis <br /> for knowledge of the waste . <br /> CORRECTIVE ACTION: Provide a diagram and list of all hazardous waste streams that go to the hazardous waste <br /> treatment system before they are mixed together in common piping . For each hazardous waste stream provide <br /> documentation that demonstrates their Title 22 hazardous waste characteristics and the amounts of each waste <br /> stream generated monthly. Documentation may include sample analytical results , material composition information <br /> that list the ingredients and Safety Data Sheet information . Also provide this documentation for the hazardous <br /> waste solids that the treatment system generates and is disposed of as hazardous waste . Keep a copy of this <br /> information in the facility file and send a copy to the EHD . <br /> Page 3 of 3 <br />
