Training records must show the required training elements (see 1, 2, 3, and 4 below) have been <br /> addressed. You may refer to the attached sample training document. <br /> 1. Familiarity with the emergency response plans and procedures in the event of a release or threatened <br /> release of a hazardous material; (Emergency Response/Contingency Plan familiarization). <br /> 2. Immediate notification contacts to the appropriate local emergency response personnel and to the <br /> unified program agency; (spill reporting). <br /> 3. Procedures for the mitigation of a release or threatened release to minimize any potential harm or <br /> damage to persons, property, or the environment; (spill response). <br /> 4. Evacuation plans and procedures, including immediate notice, for the business site; (evacuation <br /> training). <br /> • Violations 2, 3, 9, and 12 will remain open until violation 5 and 6 have been successfully addressed. <br /> The violation due date is today,January 10th; all time spent attempting to obtain compliance after the violation due <br /> date has passed may be billed at$152 an hour. <br /> As of September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
