PHS/EHD has <br /> prepared the following comments for your consideration. <br /> Groundwater samples were collected on June 2, 1994 from all monitoring wells containing sufficient <br /> groundwater to sample, MW5, MW4, EA5, and EA6. For some undisclosed reason an additional <br /> sample was collected from MWS on June 27, 1994. Please be informed that PHS/EHD was not <br /> notified 48 hours in advance of the June 27, 1994 sampling event and was unable to obtain the reason <br /> for the re-sampling from your consultant. <br /> Your correspondence indicated that a closure report was forthcoming. PHS/EHD requested in <br /> correspondence dated June 20, 1994, that a closure summary report be submitted. Since you have <br /> indicated that groundwater monitoring has be suspended pending closure, please submit by October <br /> 31, 1994 a closure summary report as outlined in the AAppendix_B_ (formerly "Addendum to Appendix <br /> A") of the Tri-Regional Recommendations. <br /> PHS/EHD looks forward to evaluating this site for closure.
