Grewal's Market Page 2 <br /> 4100 East Fremont Street March 16, 2011 <br /> Stockton, California 95215 <br /> The top three feet of the well shall be drilled out with at least the same sized auger as <br /> originally utilized to install the well; the soil around the cutoff casing shall be removed to <br /> a depth of an additional six inches below the new "top" of the cutoff casing. The grout <br /> sealant shall spill out of the top of the well to form a mushroom cap seal, with at least 12 <br /> inches of neat cement grout poured 6 inches above and 6 inches below the top of the <br /> cutoff casing. <br /> • Groundwater displaced to the surface during the grout installation shall be pumped out <br /> of the excavation and containerized for proper disposal. <br /> After the EHD has approved the well destruction scope of work proposal, submit a Well and <br /> Boring Permit Application form and the appropriate fee for each parcel. The permit fee for the <br /> destruction of each well is $122, along with a $366 inspection fee. A review of the site records <br /> indicates that four monitoring wells are installed at your site.
