NOTIFY MUNICIPAL SERVICE CENTER , Nick Williams Water/Wastewater Supervisor, (209) 333-6740 , regarding damaged or destroyed public water, sewer, storm services and mains ,, facilities and structures ; 13 . Public streets to remain open at all times . Precautionary measures shall be taken with construction vehicles and equipment that may create sight-obstruction for traveling public . Extraordinary measures in traffic control to be exercised in proximity' s of street intersections and those with high traffic volumes ; 14 . Traffic control for work within the right-of-way, including overhead operations , shall be per Caltrans Traffic Control Manual , current issue . One-way traffic requires flaggers ; 15 . If street closure required due to scope of work , detour plan will be submitted to City of Lodi Traffic Division , Public Works , Dept , 333-6706 , prior to full street closures . 16 . On projects employing pneumatic or hydraulic earth -piercing tools , interior video inspection of city underground facilities crossed near the new installation will be provided by the applicant at no cost to the City. Copies of the video tapes will be provided to the City. In lieu of videotaping , AT&T may elect to be responsible for correction and repair of any subterranean damage it caused to city facilities in performing work under this Permit for a period of ten ( 10 ) years commencing on the date work was performed under this Permit. 17 . Vaults set at back-of-walk shall be set at 2 % slope matching back of walk grade . 18 . Extraordinary measures in traffic control to be exercised due to proximity of street intersection and high traffic volumes . 19 . No drilling or boring in excess of 50-ft to take place without prior approval . 20 . DIRT IN PARKING LANE : Reflective barricades ; 246ot clearance from flowline ; stockpile maximum 6-foot width along resident frontage ; tarp , if windy conditions occur; street swept when conditions require ; no flushing or sweeping into city storm drain system ; sandbags , if necessary ; no obstruction of driveways ; stockpiling at applicant' s residence frontage only, etc . Length of time : few days is reasonable , a week at most ; 21 . Screen filters , rock/sand bags with filter screen material , or approved equivalent shall be placed at all catch basins and effectively maintained ; ( in subdivisions for a period of 1 year after acceptance) . Contractor shall place " rock bags" (approximately 12 " x 18 " and 1,5 lbs . of 1 " rock) in gutter pans at 200-300 ' intervals to isolate any sediment. Sediment to be removed weekly or sooner, if a large accumulation occurs or storm is predicted ; 22 . Work to be accomplished in a neat and professional manner. ALL TO BE RETURNED TO AS GOOD OR BETTER CONDITION . ALL DEBRIS TO BE CLEANED DURING CONSTRUCTION NOT TO BE LEFT AT JOBSiTE AND NOT TO BE LEFT ON HOMEOWNERS PROPERTY WHERE APPLICABLE .
