(Stratus) proposed the destruction of <br /> monitoring wells MWA , MW-3, and MW-4 at the subject site . These three wells have not been <br /> sampled since 2010 and groundwater monitored by these wells has historically low to non-detect <br /> concentrations of contaminants of concern . The EHD approves of the destruction of MW-1 , <br /> MW-3, and MW-4 with the following conditions: <br /> a As the wells are located in an area of known contamination , the wells shall be destroyed <br /> by removing all material within the original borehole (including the well casing , screen , <br /> filter pack, annular seal , and conductor casing) ; <br /> 4 Use 8-inch diameter augers or larger to over-drill MW-1 and MW-3 to 36 feet below <br /> surface grade (bsg) , and MW-4 to 20 feet bsg ; <br /> a The cleared borehole is then to be sealed with neat portland cement (grout) from the <br /> bottom upward , through a tremie pipe or equivalent which extends to within 5 feet of total <br /> depth , to the depth of 3 to 5 feet below surface grade; <br /> a Mix grout at a ratio of 5 to 6 gallons of clean , potable water per 94 pounds of grout; <br /> a All investigation-derived wastes (ON) , including soil cuttings, debris, and groundwater <br /> generated from soil borings and well installations, shall be containerized for proper <br /> disposal and the containers labeled to characterize the waste: hazards , composition , <br /> generation site address, generator's name , and date of accumulation ; and <br /> ® A well destruction report describing well destruction field activities and including the <br /> receiving facility-signed manifests for the disposed ID\N must be submitted to EHD . <br /> To proceed with well destruction permit issuance the following must be submitted to the EHD to <br /> the attention of Gabriel Gonzalez (please see page 2) : <br /> a EHD Well and Boring Permit Application form; <br /> ® Master File Record form (MFR) ; <br /> a Licensed Contractor's Declaration (LCD) ; and <br /> a A fee of $390 (3 x $ 130) . Additional inspection time spent beyond the initial pre-paid fee <br /> will be billed at the current hourly rate of $130 . <br />
