Mr. Dave Isola - 2 - 21 June 2012 <br /> analysis of EHC from the manufacturer, therefore, Genesis Engineering does not need to <br /> provide an additional analysis. <br /> An annual fee of$3,422 is required to cover administrative costs of the General Order. <br /> Please provide the following remaining items to complete the Notice of Intent. <br /> • A commitment to develop a contingency plan within 30 days of a request from the <br /> Central Valley Water Board. <br /> • A check for$3,422 payable to the State Water Resources Control Board and provided to <br /> Central Valley Water Board staff. <br /> When Genesis Engineering provides a commitment to prepare a contingency plan and the <br /> General Order annual fee, Central Valley Water Board staff will provide Isola and Genesis <br /> Engineering with a revised draft Fact Sheet, a draft Monitoring and Reporting Program, and a <br /> draft Notice of Applicability. When Isola and Genesis Engineering and Central Valley Water <br /> Board staff are in concurrence with the language in these documents, then Genesis Engineering <br /> will circulate the fact sheet, and Central Valley Water Board staff will post the documents on the <br /> internet. These actions will begin the 30-day public comment period. Any comments that are <br /> received will be addressed, and the project may be revised, if necessary. <br /> After the public comment period, the Central Valley Water Board Executive Officer will review <br /> the application, and when the Executive Officer issues the Notice of Applicability to Isola and <br /> Genesis Engineering, then Isola and. Genesis may proceed with the EHC injections.
