The most recent financial <br /> responsibility documents found on site are dated 8/29/2018. <br /> REGULATION GUIDANCE:A copy of current financial responsibility documents are required to be maintained on <br /> site. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a <br /> copy on site. <br /> This is a minor violation. <br /> 107 CCR 2632(d), 2641(h),2711(a) Plot plan/site map not submitted or failed to completely show where monitoring is <br /> performed. <br /> OBSERVATION:An accurate UST Monitoring Site Plan was not submitted. <br /> -The description and location of the oil water separator on the map is not current. The 1,000 gal oil water separator <br /> (tank#7) was removed on 1/2/2020 and a flow through system was installed at a different spot. <br /> REGULATION GUIDANCE:A site plan must be submitted identifying the locations where monitoring will be <br /> performed. The site plan must show the general layout of tanks and piping and clearly identify locations of the <br /> following equipment, if installed: 1)monitoring system control panels; 2)in-tank liquid level probes (if used for leak <br /> detection); 3)devices monitoring tank annular spaces or vault;4)devices monitoring product piping; 5)devices <br /> monitoring fill piping; 6)devices monitoring vent piping; 7)devices monitoring vapor recovery piping; 8)devices <br /> monitoring vent/transition sumps; 9)devices monitoring under-dispenser containment; 10)line leak detectors; and <br /> 11)devices monitoring any other secondary containment areas. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a minor violation. <br /> 110 CCR 2637(e), 2637(f)Failed to submit"Secondary Containment Testing Report Form"within 30 days after the test. <br /> OBSERVATION:A repair secondary containment testing was was performed on3/3/2020 and a test report was <br /> submitted on 4/8/2020, 36 days late. <br /> REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are <br /> performed in a timely manner. <br /> This is a repeat violation, Class ll. <br /> FA0006672 PR0505264 SCO01 05/05/2020 <br /> EHD 23-01 Rev.9/20/2019 Page 4 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
