The following is an itemized list of hazardous materials violations that have not been addressed for POPEYE'S as of February 10, 2020. Open violations from October 03, 2019 inspection Violation #10 -Failed to submit revisions to business plan within 30 days of substantial change in operations. During the inspection, it was found that the business is handling carbon dioxide and the updated information has not been submitted in the chemical inventory. Within 30 days of one the following events, business shall electronically update the information submitted to the California Environmental Reporting System (CERS): -A 100 percent or more increase in the quantity of a previously disclosed material. -Any handling of a previously undisclosed hazardous materials -A change of business address, business ownership, or business name. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#11 -Failed to provide or document initial and/or refresher training to appropriate personnel. Employee training record from 2017 were available for review. Please send me updated employee training records for 2019. The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately provide employee training for appropriate personnel and submit a copy of the training records to the EHD. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#12 - Failed to have the business plan readily available to facility or EHD personnel. Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency response plans and procedures, the inventory of information required by this article, and the site map required by this section shall be readily available to personnel of the business or the unified program facility with responsibilities for emergency response or training pursuant to this section.
