The EHD had directed the submittal of a well destruction work plan by letter <br /> dated 16 January 2013. <br /> In the work plan, Cardno ATC proposes to destroy monitoring wells MW-1, MW-2, MW-4, MW-5 <br /> and MW-8 by removing the well components by over-drilling the wells; and MW-3, MW-6 and <br /> MW-7 by pressure grouting. <br /> The EHD concurs with the well destruction methods proposed by Cardno ATC with these <br /> conditions: <br /> • Use twelve-inch diameter, or larger, hollow stem augers to over drill monitoring well <br /> MW-8 since records indicate that MW-8 was installed using twelve-inch diameter augers; <br /> • Apply twenty-five pounds per square inch (psi) of liquid grout pressure for a minimum of <br /> ten minutes when pressure grouting MW-3, MW-6 and MW-7; and <br /> • Notify the EHD at least two weeks prior to the commencement of the well destructions. <br /> Please submit a Well and Boring Permit Application for the well destructions, Master File <br /> Record form, and appropriate fees for this site. The permit fee for the destruction of each well is <br /> $125.00, along with a $375.00 Inspection Activity fee per parcel. Since eight yells currently are <br /> installed, a fee of$1,375.00 will be required for the well destruction permit. <br />
