In the event of an evacuation, the <br /> assembly point is the access gate to the site. <br /> oo Incidents — report all incidents (any unplanned or unexpected event that results in personal injury, <br /> property damage or environmental release) and "near-miss incidents" to your Supervisor and the Site <br /> HSO. Near incidents COULD HAVE been an incident, but didn't because of a slight change in <br /> conditions or luck. However, they have the same causal factors as an incident, so it is just important <br /> to investigate them for identifying solutions to prevent recurrence and share lessons learned. <br /> oo Communications — the response for anyone from the general public (e.g. media, workers from <br /> adjacent properties, etc.) inquiring about the project is: "I'm sorry, but I'm not the right person to <br /> answer your question. If you contact the project manager Tom Blaney he will be able to help you". <br /> Safety, health, and other hazards at the site <br /> oo Exposure to contaminated soils and groundwater posses the greatest risk during site activity. Heavy <br /> equipment (Drill rig) will be in operation during the investigation and should never be approached <br /> unless signaled by the operator or the equipment is not running. <br /> oo Exposure potential is minimal but possible during soil and groundwater investigations. Multi-gas <br /> meters will be used to monitor the air five to six feet from the ground to simulate the workers <br /> breathing zone. If multi-gas meter readings are above 5 ppmv for 30 seconds in the breathing zone, <br /> then Draeger tube readings and Chromosorb 106 tubes will be taken to determine if the vapors are <br /> Benzo(a)pyrene (BAP) or Benzene. If benzene levels exceed 0.5 ppmv, then personnel will be <br /> required to wear respiratory protection. If VOC readings consistently exceed 25 ppmv, then work <br /> will be stopped until conditions can be brought back into compliance. A water truck will be present <br /> onsite at all times for dust suppression. <br /> Proper use of personal protective equipment <br /> oo Minimum PPE - hard hat, safety glasses with side shields, steel toe work boots and long sleeve shirt. <br /> Tinted safety glasses are not permitted during overcast weather, after sundown or inside buildings. <br /> High visibility vest/coat (with reflective stripes if working after dusk) is required when regularly <br /> exposed to vehicular traffic. Any high visibility vest/coat other than a traditional traffic vest must be <br /> pre-approved by the Safety Manager. <br /> oo Hearing protection—when working in close proximity with heavy equipment or you need to raise your <br /> voice when speaking to someone 3 feet away. <br /> Work practices by which the employee can minimize risk from hazards <br /> B-1 <br />
